What Cagrilintide is
  • A peptide analogue of amylin — the hormone the beta cells of the pancreas secrete alongside insulin.
  • Human amylin tends to aggregate into fibrils; the sequence here is deliberately modified so that the molecule stays soluble.
  • A fatty acid chain attached to a lysine keeps it bound to albumin, which in studies extends how long it stays in circulation.
  • Unlike the incretin analogues (GLP-1, GIP) it does not act on incretin receptors — this is a separate pathway that the literature combines with them.
  • Supplied as a lyophilised powder for reconstitution.
Documented mechanism
  • Published work describes binding to amylin receptors, which are formed by the calcitonin receptor in complex with the RAMP1–RAMP3 proteins.
  • Signalling is described in the brainstem — the area postrema and the nucleus tractus solitarii — that is, outside the blood-brain barrier.
  • In models it is associated with slowed gastric emptying and with suppressed food intake through a satiety signal.
  • The amylin pathway is distinct from the incretin pathway, so their combined action is a research branch of its own.

Research context
